Refer a Patient

We always welcome your referrals and appreciate your confidence in our care.

Patients are asked to bring the following items to their appointment:

• Any x-rays given by their dentist
• Insurance cards/forms
• A list of medications being taken

Patients under the age of 18 must be accompanied by a parent or guardian.
